Community Supports and Services

  • Community Habilitation

    Community Habilitation services focus on teaching the skills needed to help a person become as independent as possible at home and in the community. Areas of focus may include completing household skills such as grocery shopping, maintaining a clean home, meal planning, and budgeting as well as community-based skills such as utilizing public transportation and community safety. Services are available for people who live independently, with family, or in a certified residence.

  • Intensive Parent Training

    This group works alongside parents with disabilities to teach childcare skills, and daily living skills. Including cooking, cleaning, and being a parent advocate. Programming can occur in small groups or one on one settings.

  • Children's Health Homes

    Case management services for children up to 21 years of age, who have a chronic health condition, serious emotional disturbance, or complex trauma. Care Managers provide linkage and referral and support the family unit through person-centered planning with medical providers.

  • Children's Home and Community Based Services

    These respite community habilitation and caregiver & family support programs are designed to provide services to youth at risk of admission to higher levels of care. The goal of these programs is to identify the area of need and work to prevent the need for further restrictive services allowing children to remain in the family home.

Respite Services

  • After School Respite

    After school programming is available to students living in the Maryvale, Williamsville, West Seneca, and City of Buffalo school districts. Program operates in alignment with the district’s calendar.

  • In Home Respite

    In Home Respite provides support for families and caregivers as it allows them to have a break in caregiving while a trained staff person provides supervision for their loved one. Services occur sole in the person’s home, giving family members the opportunity to leave the home to run errands or remain in the home and take time to relax.

  • Recreation Respite

    This community based program typically gathers at a hub site before enjoying a wide range of events and activities available in the Buffalo community. A monthly calendar outlines available activities and participants are invited to attend events based on their interests.

  • After Program Respite

    This site based program is designed to extend the length of the program day by providing additional support and supervision. This program is available to adults to engage in a variety of activities such as social groups, technology usage, music, and crafts to name a few.
